Call 1-800-923-7021What Is Case Management?
Case management is a collaborative process in which a dedicated case manager works with you to assess your needs, develop your treatment plan, coordinate services, and plan for your transition out of residential care. At Brighter Day Detox, every client is assigned a case manager upon admission who serves as your primary point of contact and advocate throughout your stay.
Treatment Planning
Your case manager collaborates with your clinical team to develop and update your individualized treatment plan — ensuring your goals are clear and your care is coordinated.
Benefits & Insurance Navigation
We help you understand your insurance benefits, navigate coverage questions, and connect you to financial assistance resources when needed.
Community Resource Linkage
We connect you to community resources including sober living homes, outpatient programs, employment services, housing assistance, and support groups.
Family Communication
With your consent, your case manager can serve as a point of contact for family members — providing updates and coordinating family involvement in your treatment.
Discharge Planning
From day one, we're planning your next steps. Your discharge plan includes step-down treatment recommendations, sober living options, ongoing therapy, and community support resources.
Crisis Intervention
If challenges arise during your stay, your case manager is your first point of contact — helping to de-escalate, problem-solve, and keep your recovery on track.
Discharge Planning
A strong discharge plan is one of the most important factors in long-term recovery success. We begin discharge planning from your first day in treatment — not your last. Your plan may include step-down to outpatient treatment, sober living placement, ongoing individual therapy, medication management, support group participation, and connection to community resources in your home area.
